[Cefpodoxime Proxetil 200 mg + Clavulanic Acid 100 mg]
Sefacef-CV combines Cefpodoxime Proxetil, a third-generation cephalosporin, with Clavulanic Acid, a beta-lactamase inhibitor, to provide enhanced antibacterial action. It is ideal for a wide range of community-acquired infections in both adults and children.
Antibacterial Spectrum:
- Active against Gram-positive and Gram-negative bacteria
- Effective in empirical therapy of diverse infections
- Not active against Pseudomonas aeruginosa, Enterococcus, Bacteroides fragilis
- Clavulanic acid extends the antibiotic spectrum by neutralizing beta-lactamases
Mechanism of Action:
- Cefpodoxime Proxetil is a prodrug activated in the intestines
- Exerts bactericidal action by inhibiting bacterial cell wall synthesis
- Binds preferentially to Penicillin Binding Protein 3 (PBP-3)
- Clavulanic acid protects against beta-lactamase degradation
Indications:
- Gonorrhea
- Community-acquired pneumonia
- Sinusitis, acute otitis media, pharyngitis, bronchitis
- Intra-abdominal infections
- Skin & soft tissue infections: abscess, atheroma, furuncle, carbuncle, infected wounds, cellulitis
Warnings:
May disturb gut flora, potentially leading to diarrhoea
Side Effects:
- Common: Diarrhoea, nausea (4–15%)
- Rare but serious: Anaphylactic reactions, pseudomembranous colitis, neutropenia (with >10 days use)
Pharmacokinetics:
- Route: Oral
- Absorption: ~50%; enhanced with food
- Half-life: 1.9 to 3.7 hours – allows twice daily dosing
- Metabolism: Minimal; non-absorbed drug excreted via feces
- Excretion: 29–33% excreted unchanged in urine within 12 hours
Contraindications:
- Known hypersensitivity to any component
- Pregnancy and lactation
Precautions:
- Adjust dose in renal impairment:
- ↓ 50% for Creatinine Clearance 10–30 mL/min
- ↓ 75% if <10 mL/min
- Caution in patients with penicillin allergy
- May cause superinfections with prolonged use
Drug Interactions:
- Reduced efficacy: Antacids, H₂ antagonists, anticholinergics
- Increased effect: Probenecid
- Avoid with: Diuretics, other nephrotoxic agents
Dosage and Administration:
1 tablet twice daily, or as directed by the Physician (Healthcare Provider)
